Nashville-based NaviHealth has signed a deal with Cigna-HealthSpring to manage post-acute care services for about 100,000 Cigna-HealthSpring members in the mid-Atlantic and Pennsylvania regions, the companies announced today.

With sights set on improving patient outcomes and reducing costs, a partnership between Cigna-HealthSpring and naviHealth aims to help hospitalized seniors navigate post-acute care options upon discharge.
Under the program, naviHealth’s team of nurses and licensed therapists will engage and assess Cigna-Healthspring members in the hospital to educate them and their caregivers on their options for post-acute care and rehabilitation.
