Health Alliance Medical deal grows to include Medicare Advantage members
Post-acute care delivery company naviHealth today announced an expansion of its partnership with Health Alliance Medical Plans, an Illinois-based health insurer with about 300,000 members. As a result of the agreement, naviHealth will offer care management services to Health Alliance’s Medicare Advantage members. Financial terms of the deal were not disclosed.
Under the expanded program, naviHealth’s care managers will work with patients, caregivers and providers to set personalized rehabilitation goals and measure progress towards the goals until a patient is successfully transitioned back to the community. The partnership also includes the use of naviHealth’s proprietary clinical decision support technology. Patients will work with field-based care managers across the post-acute care continuum, with the goal of preventing costly hospital readmissions.
This new agreement will go into effect May 1.

Nashville-based naviHealth has been managing post-acute care in the skilled nursing facility setting for Health Alliance since 2010. The company is backed by Welsh Carson Anderson & Stowe, Select Medical and Universal American.
